Job Summary Responsible for the delivery of professional, complex services to patients and patient's families within an interdisciplinary team setting.

Working Conditions Primarily Works in a well-lighted and air-conditioned environment with period of heavy workload and stress. This role may include working in less-than-ideal home conditions, which can include exposure to extreme temperatures and environments that may not meet typical cleanliness standards such as clutter, unkept surfaces, and homes with pets. Works in various conditions.

Performance: Position Specific Essential Functions - Assess the social and emotional factors in order to estimate patient's capacity and potential to cope with problems of daily living.
- Assist patients and families to understand, accepts, and follows medical recommendations and provides services planned to restore patients to optimum social and health adjustment within their capacity.
- Assist patients with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
- Utilizes resources, such as family and community agencies, to assist patients to resume life in the community or to learn to live with any disabilities they may have.
- Develops and maintains working relationships with other departments, agencies, patients, and their families.
- Participates in Patient Care Conferences and Utilization Review Meetings.
- Maintains current information on community health and welfare resources in order to ensure appropriate referrals
- Adheres to department and hospital policies and procedures. Follows work schedule and demonstrates good attendance.
- Communicate with patients, families, staff, and others in a professional and service-oriented manner.
- Utilizes good judgement in the performance of duties.
- Attend meetings and in-service as required. Attend appropriate conferences and workshops to enhance career growth and development.

Qualifications Education
- Required
- High School Diploma, GED, or equivalent
- Master's degree in social work
Experience:
- Two years of experience in Social Work Services
- Two years of experience in Counseling
Certification/Licensure:
- Required
- Licensed Master Social Worker (LMSW) OR Licensed Clinical Social Worker
- Must be current in the State of Texas
- Basic Life Support (BLS) Certification
- Must obtain within ninety (90) days of start date
- SAFE Approach Level III Certification
- Must obtain within six (6) months of start date
